I've been using such forms to pick up dog poo for some time. Let's count the ways.... hospital nurse said I was black, then when I insisted that my ethnicity had nothing to do with being sick,now I'm white, US says I'm mixed race, Department of Labor says I'm of the bitch race for not stating my race,etc.
