Yep...big red flag time. As I stated earlier...I am getting into this late in the game. I don't have nearly as much information and particulars as many of you do on this thread. Nurse Carla's statement caught my eye, and I am looking at it from an administrator's position and from what a staff members position should ideally be. If Florida is anything like CA and I'm sure it is. Staff are mandated reporters. Period. No if's and's or but's. Unfortunately, sometimes this does not happen.

As a consultant, I went through 1000's of charts in LTC facilities. I would say a high percentage of charts where train wrecks.
Poor or missing documentation, pages missing, falling out or in the wrong section. Pages 'replaced' or forged. Weights that hadn't been recorded for six months suddenly appearing (same with other tests etc).
