I believe you are misinterpreting what he meant.
He meant I believe “expand it to everyone who needs coverage”.
Not “expand to everyone”.
That is a not insignificant difference.

And he said: “This is an un-Republican thing for me to say..”
And.. given his past statements, I think your interpretation is more a stretch.
Putting together all of the published statements of Trump, it sounds like to me that, he believes a base level of care that does cover everybody, but a private system for anything above base care.
