That would be the capitalist way being it's insurance companies making the call. But it has nothing to do, directly, with what type of economy is in place...and often what type of government is in place.

I suppose you could add a couple of rules to Obamacare and Medicare to ensure all folks with heart failure (90+% lifestyle caused) get rehab. But most will not be interested otherwise they wouldn't need it.

If unsure, maybe just ask before assuming the worst. Its hard when its just text only and no sound and facial expressions with it.
