One of my humorous stories was when I took one of the old school fanfold continuous reports off a line printer and laid it on someone’s desk. The lady started flipping through the pages and said to me... You printed it backwards! I took the report and flipped it over. She was humiliated.

Upper management came up with a QA document where the Functional Area would sign off on projects before they went live. It said that the tests showed the system was working according to the specs.
The problem was that no one would sign it because they didn’t look at test results for fear of finding something wrong. They would push the paper all over the office until it arrived on my desk unsigned. I would hand it to my manager and it would go around again with the same result.
