I must disagree with your assessment of Brad Pitt in Fight Club. Pitt was an excellent choice for the part and did a great job of acting. For an "eye-candy" actor, he's actually quite talented. Far more talented than fellow eye-candy actor Tom Cruise.
Miscast actors
Orlando Bloom in "Kingdom of Heaven"
Viggo Mortensen in LOTR
Keanu Reeves in "Much Ado About Nothing"
Keanu Reeves in any movie that requires him to speak dialogue beyond "whoa" and "I'm Neo."

Actually the worst and most disappointing was Robert Redford as the Horse Whisperer.
Being a horse owner/enthusiast and having read the book I was anxious to see the movie.
It was awful.
And Redford was even worse.

Clint Eastwood in Bridges of Madison County. The bathtub scene still gives me nightmares.Excellent call. He was horribly mis-cast in that movie. What was he thinking when he took that role.
