Just as a related fight is hitting the U.S. Supreme Court, the lower 6th U.S. Circuit Court of Appeals has confirmed that talk therapy for minors with gender confusion issues is, in fact, protected speech.
"The Sixth Circuit has rightly ruled that laws restricting counselors and clients to only one viewpoint violate the First Amendment. Talk therapy is speech, and the government has no authority to restrict that speech to just one viewpoint," explained Mat Staver, chief of Liberty Counsel, which repeatedly has fought such First Amendment violations.
"The ruling allows therapists to engage in professional, compassionate counseling giving children a path to alleviate their mental health issues. Counselors and clients should have the freedom to choose the counsel of their choice and be free of government censorship."

I read it as it is acceptable for therapists to tell kids “You aren’t really a boy; you are actually a girl. If we chop your nuts and pecker off and cram you full of hormones, you will be a lot happier. Trust me on this.”

Allowing both sides to be presented is one thing, getting the counselors to do it is the bigger problem.
The tide seems to be turning. In my locality, the large City of Arlington, Texas recently REVERSED its LGBT protection ordinances. That doesn't happen very often.
You're missing the point. Only the left was targeting Christian counselors, telling them that they COULD NOT steer a minor away from identifying as "gay" or "trans." They could only confirm these identities. Christians were not telling counselors what they could tell their clients, even the despicable queer agenda stuff.
People can now pick their own counselor for unwanted same-sex attraction counseling and not worry about the guy being fine out of business or losing his license to practice.
So why isn’t talking children into sex not free speech? Rhetorical speaking, of course.

Then prosecute groomers. But limiting what a therapist can talk about is the wrong track for this type of mental illness. I presume it could be helpful to talk through all these different feelings and have them think about what the true end result would be if they chopped off body parts and what if they wanted to change back when they were older.
I’m just saying limiting it would be very odd and hard on therapy because they would be uncomfortable talking down these paths for fear of prosecution. And keep in mind the witness against you would be a mentally ill person.
Protected ? yes.
Private (as in don’t tell your parents) ? Hell no.
Is that what the Supremes are hearing.
Does this ruling also apply to pronouns and dead-naming?
