Things like this are not a new practice. Insurance companies are going to protect their bottom line for themselves or the client they are servicing policies for - if there is a treatment available that will reduce their exposure, they will promote it. Given the enormity of the exposure from the jump in hospitalizations from people getting infected with COVID19, the $125 figure is probably accurate. More people being vaccinated greatly reduced the exposure to these insurers by reducing hospitalizations and serious illnesses. This is a sensationalized article from a dubious source trying to act like they've uncovered something totally unheard of and they've broke something big. They didn't.

Now...that said - this is a questionably ethical practice, of course, to pay doctors more to provide certain treatments. But this is not new or unique practice arising out of COVID19.
