My understanding is citizens can exit and enter the US as we please.
Non-citizens require vaccination, just as you said. Well, not “refugees.” That’s different or something.

I did not travel recently; as far as I know no jab requirements for citizens or immigration status holders. Foreigners are required to be vaxxed, albeit don’t know it is only applicable to tourists or business invitees as well.

If you're a citizen being unjabbed isn't an issue. However,for getting into a foreign country it might be. If you have a Green Card I would think you'd be admitted..but could be wrong.
