There are lots of congenital heart defects.

In this case one kid had cardiac hypertrophy which is genetic half of the time and the other kid is suspected of having arrhythmogenic cardiomyopathy which is an inherited genetic condition.

He had scarring on his heart that almost certainly preceded the vaccination.

Both of these conditions can be asymptomatic until a fatal arrhythmia occurs so it isn't surprising that they didn't have a history of problems.
