I kind of like the Japanese custom of leaders being ashamed of themselves if they fail—enough to fall on their swords. Our leaders have no shame whatsoever about their failure/incompetence.

Failing that, at least resign. Mark Steyn wrote yesterday that following the Suez fiasco in the 50s, high officials resigned. There used to be this quaint idea that if you failed miserably in your job and embarrassed your nation, the honorable thing to do was to resign.
