The US has long had medical supplies exemptions for Cuba. They get special export/import license for what they buy here or what is donated by the US.
Castro got a lot of his medical stuff from Marxist Trudeau’s Canada, some from England, Spain, the Communist Bloc, Marxist Nicaragua, Venezuela, possibly Marxist Bolivia, and even Red China. Might even have gotten some from Marxist nations in Africa including South Africa (ANC/CPSA), Angola, once occupied by Cuban troops, the same for Mozambique, Cape Verde/Guinea Bissau, Bukina Faso, Libya, and maybe Iran (over the decades).
