Its not merely a poorly run or efficient government program.
It was planned, designed and implemented as outright theft. CIA probably needed cover to funnel money to some assets in-country. Afghan cronies probably knew who would get how much in advance. Now the money is in banks in Dubai.

Hell, you could’ve paid them $1 million each, given me a 10% commission for figuring out the plan, and STILL saved a boatload of money.
We need to start making federal employees responsible for the money they spend - that way, they might actually give a rat’s ass.
