We all have been in vehicles. You are not thrown about during a ride on a city street. I believe he tried to escape and ran into the van door and ended up killing himself.

Put a reporter in that same van and drive on the same streets. He can wear padding if he wants to though it will not be needed.

Escape how? His wrists and ankles were shackled and he was in a locked compartment. You think maybe he thought if he hit the door often enough with his head he'd punch right through it?

If the driver wants you to be thrwn around during a ride on a city street, you’ll be thrown around during a ride on a city street. Especially if you’re in wrist & ankle restraints, with no seatbelt, on a metal surface.

if he was withdrawing from heroin anything could happen. Drug seekers have been known to hurt themselves to get pain meds.

But I still wonder about the driving. No, we aren't thrown about by riding on a city street, yet the coroner surmised

he may have gotten to his feet and was thrown into the wall during an abrupt change in direction

While driving on city streets you usually don't experience "abrupt changes in direction." Of course the bottom line is, why wasn't he buckled in as the rules state? Has there been any explanation of that?

We have all NOT been in vehicles with our hands and feet shackled... NOT buckling him in while shackled is criminal. Period.
