Teddy Roosevelt was widely known by his initials, apparently the first president to be so widely identified this way.

I’d guess the use of FDR or LBJ (or TR) actually originated with their campaigns.

Yes TR preferred to be called that and did not like to be called Teddy.

TR also started the “deal” series with “The Square Deal”.
