Here is a slightly larger version, but the important hand written stuff is mostly too unclear to be legible.

The more I read that, the stranger it becomes. The typewritten lines are done by someone unfamiliar with Mexican Spanish, or semi-literate. But then there are words that would be totally unfamiliar to native Latin illiterate or semi-literate speakers.

From top to bottom, the typewritten lines are:

"Why did you abandon (leave) your country?"
"Due to poverty and misery"
"Are you afraid of your government and afraid to live in your country?"
"Are you afraid of extortions by crooks?" (last word unclear)
"Have you been extorted (feminine) not only once but more than once?" (the use of the word 'solo'is usage by semiliterate Spanish speakers, or an American with limited control of Spanish)
"Do you have family in the United States?"
"Have you previously lived here?"
"Is this the first time that you enter this country?"
"Through which (part of the) border did you enter?"
"Did you swim across the river?"
"Did anyone tell you that if you brought minors with you, you could enter the United States?" (the use of the phrase 'in menor' is weird; not the way Mexicans or people farther south speak. More a gringo poor attempt to write in Spanish.) Image and video hosting by TinyPic

My best guess after writing this up is that this sheet was prepared for use by the American border patrol, and subsequently duplicated and somehow smuggled across for use by the coyotes across the border.

The hearts' scattered around the sheet suggests this was in possession of a young female illegal. The handwritten lines that can be read clearly are as follows...

"Because I had no one to help me"
The "Si" and "No" answers are self explanatory.
"Whom did you live with?"
"With my uncle Pero(?)" what follows is illegible.)
"To my father where he is living."
"I do not know (him or her)" (the rest is illegible.)

The handwritten notes give us the most insights.
