I'd have to argue that your proposed categories (3) and (4) are (IMHO), subsets of my category (2). That is, category (2) may be divided into the stupid, the lazy, and the willfully ignorant. Those are valid distinctions.
Your category (4), my category (2.c) seems especially in evidence here today.
