Seems like they lost cooling of the fuel. I know not much about reactors but 4 GE reactors built in the same place and all 4 having the same problem looks like a bad design by GE.

The reactors are 40years old, so maybe not a bad design, but certainly an out of date or obsolete design. The basic design is probably ok, but would be completely bulletproof with upgrades and additional backup redundancy. When the immediate emergency is dealt with I will be interested to see if they replace the undamaged reactors or upgrade them with what they haved learned from this.
