Every night I load the 16th round into the chamber and the first thing I do when I wake is to clear the chamber. You may already be aware, but be watchful for bullet setback .

Even the slightest crimping can cause problems, and I've tried a variety of quality and it's not the determinant. It's totally a combo of any and all factors it seems--but for me, spring tension is a biggy starter, along with the specific gun. I rotate a lot and rotate out a lot, just to be safe. My newer Beretta Storm Px4 .45's magazines are almost impossible to fully load in the first place...getting easier though.
