Democrats can count on at least some Republicans to stand on principles when the Pubs are in power, so Dems are safe to ignore them when they are. And the Democrat leadership can retaliate aganst members who don't play ball by removing them from committee positions. The Pub leaders can't, due to their party rules. So the Dems have unity and the Pubs are fractured.

They can try to do this but I suspect a few moderate senior dems won’t go for this type of power play. Payback will suck at some point and the repubs will have the health care push through to point to.
