I haven't seen the picture in question, but I can see how some people might find it offensive, but I am not sure I would.
I took an example of say, Obama walking with a daughter on each hand, and had the heads of Barbara Boxer and Nancy Pelosi Photoshopped on their shoulders...I had a hard time getting worked up about it.
OTOH, I wouldn't do any Photoshops of the kids of any public figure. We should be able to poke fun at the parents without using the kids as props. Parents are fair game, especially Barack and Michelle.

they put a pic of some politician over little trig’s face while sarah is cuddling him......what irks me most about it is that they have used trig like he is not even human from the gitgo....first suggesting that he was bristol’s child and now this.....he is an innocent and the using of him degrades him in a way they do not understand. it has to be that he is a down’s baby and they loathe him, there is no other explanation.
