It's a real problem for a forger to get hold of a long BC form circa 1961 -- maybe getting the same old paper is easier to print a duplicate form on it. The forger would need a doctor's or mid-wife's signature, et al. Forging the signature of a dead doctor or mid-wife would be the way to go. The forger would need an old typewriter and ribbon to go with it. After the forger completed a forged professional looking document. He'd have to switch it with the real one. Get someone on the inside to switch it and to get rid of any supporting documents.
