Personally, I think it’s a fake.

With the exception if the seal, all the fonts are the same family, though not the same size. The spaces to be filled in the form should have been filled in with another manual type-writer or perhaps a selectric or some other device for applying ink to paper, I would think. No type-writer or selectric or other printer would have Helvetica as a typeface—much less to apply ink to paper as pristinely as a the printed form it’s on.

And look at the border design on the left side. There appears to be some overlap of the vertical border with the top and bottom horizontal borders. That doesn’t exist on the right side. It looks like an imperfect applied computer copy-cut-past-align job.

In the 60’s, the original layout artist probably would have used Letraset or some similar applique border and cut the corners at a 45-degree diagonal, rather than overlapping the borders as bars or having them meet and at flush edge.

i tall looks like it was designed/laid-out and printed as one piece. Just my opinion.

Well, never mind. In the lower left it says (11/01) Laser or the like. Looks like the info might have been printed on a current fresh certificate using info from a database. It’s not a facsimile of an original document from the 60’s, I don’t think.
