In a web post that seems to refute the LA Times blog, Janice Okubo Director of Communications of the State of Hawaii Department of Health confirms apparently that the document is a forgery:

This is a find because it has the Obama Campaign claiming this is indeed the authentic passport of Obama. Which we know now to be a forgery...
