The Cost of Redundant Government

The USS San Francisco crashed into a mountain that the Navy's mapping & charting department didn't know was there. The NOAA, USGS, and NASA all knew it was though. If the navy simply worked with the NOAA then at least one sailor would still be alive.

How many accidents, terrorist attacks, and tragedies will it take before our government realizes that redundant organizations are not the way to go.
