This is very important to understand here, regarding the raised and underlined "th" vs. the true superscript th. Here is the difference in simple terms:

The official records show a "th" where the top of the letters do not extend above the top of normal character line. This means that a typewriter key could strike the paper just as any other key and produce this character.

The true superscript th however actually sits above the normal character line. This means that the key would not function within the same striking range as the rest of the keys.

Something that I find odd that I don't think anyone else has mention; Where's the Distribution table? on the "MFR?" At the left margin an order should go below the signature block, such as Distribution - A
or Distribution;
S1 (Personnel Records) It's normally filed in your personnel records
S2 (Security and Intelligence) Failing to do something like this can result in the administrative suspension of your security clearance
S3 (Operations and Training) Training records have to be maintained and are inspectable records. Making the Physical should/must be annotated in the training records (date of last physical) and admonition like this would red flag his training records so he could not be scheduled by ops for flight hours.

Where's that little piece?

Back in 1971 - 1973, typical typewriters (found in desk and portable models) were manual and electric typewriters. It wasn't until the 80's that electronic typewriters were commonly available. If I was typing a school paper, I would have to make superscripts which at that time simply meant they were raised ½ space above the base line. This was accomplished by stopping and use the carriage control knob to raise lower the roller ½ space. Then type the superscript letters and return to the base line.

The raised letters were the same letters used in base line text so the superscript letters were the same size. There were not separate letters for typing the superscript. In WORD, superscript letters are about ½ the size of normal font and not raised above the line but only occupying the upper portion of that line.

I don't remember any desk typewriter that had electronic capabilities. That's my 32 year old memory.

Am I wrong or would the original documents in the hands of an expert solve this in about 2 minutes. A typewriter HITS the paper and would cause disturbances in the fiber which would be readily visible. Furthermore, the Selectric typewriter did not use a cloth ribbon but a plastic ribbon. Unlike the old cloth ribbons when you typed the letter on a selectric it passed the pigment from the tape to the paper. In fact, if you got a hold of a selectric ribbon after it was used you could read all the text as transparent letters on black tape. But, looking at the original documents through a microscope should reveal if the letters were created with an impact or an inkjet.
