I don't think it was an accident at all. I truly believed he killed her deliberately for some reason. At the very least he's comparable to a drunk driver who "accidently" kills someone because they are completely and totally wasted.

He was trying to intimidate/persecute her by shooting a blank at her. No one expected a gunshot to happen, they were staging the scene, not recording. Dumbass armorer left a live round in the pistol after their recreational shooting. His intimidation attempt turned into a death and a wounded person.
