My RN wife of 81 years had never heard of that phrase.
For many decades, automobile headlights were round and may have best fit for the term headlights being turned on.
In the mid 70's, vehicle headlights began moving to square & rectangle shapes.
I would guess at that point, the phrase "headlights" lost it's identity and association with being turned on.
Does the double vax mean double dose at one time or was this her second booster?
Watch for increase in breast cancer and other cancers.

That comment was usually reserved for ladies who somehow got dust on their bosoms from working. My mother was fond of black tops at her job. She was endowed well and she would hear that from guys from time to time.
People would say “your headlights are on”!
