Catastrophic airliner mechanical failure modes are, obviously, rare, or planes would be falling out of the sky left and right.

Only one engine has fallen off a DC-10 in flight, for example.

I think they found it and fixed it (sort of).

The bum cargo door locking mechanism didn't help the DC-10, either. I don't remember how many planes suffered catastrophic decompressions before the problem was located and fixed.

Too late for the airframe to continue service as a passenger transporter, though.

But soon enough to function as a cargo plane.
