I did not have to show ID. Signed up on a site for the county health dept,and was assigned a 5 digit code. Set my appt. When I went in, I just had to give my code, no ID was requested or needed, nor for the 2nd shot.

You are correct. Under Colorado law, it is illegal for someone giving the COVID vaccine to require any sort of ID as a condition for giving it. This has nothing whatsoever to do with me nor with my actions. This is state law: ID is not only not required, asking for it is prohibited.
What if someone visited and gets vaxxxed in colorado and loses their vax card, how could that someone get a replacement? asking for some friends who lose things

Apparently you call up the location where you got it, and identify yourself (including info you gave them - there is a form to fill out, which they obviously have) and they will issue you another card. This is what I understand.
