There are some stats that stick out that may indicate a problem with their offense.
Their third down percentage was only 34.5% and they ranked 28th in the league. Their 4th down conversion rate was a little better at 41% and they were rated 17th in the league That indicates a 3rd and 4th down problem all over the field, not just in the red zone. And thats with all the backs they have.
He had a completion percentage of 76% but a yards per catch of only 6.9 yards. So it has the appearance that once he has the touch, he is trying to do it alone without interference in front of him because it looks like they threw a lot of screens and swings to him based upon the yardage and not downfield. And the backs cant do it alone in that scheme.
The three TDs I mentioned he got were the only rushing TDs all year for the team. And all three of them were one yard or less rushes: one on October 6, and two on December 1, 2019. So he could score in close, but there didnt appear to be many opportunities and a high third and fourth down failure rate. That normally indicates a problem with offensive scheme or up front. The quarterback that played the most games was Gardner Minshew and he threw all 21 touchdowns they had on the year in the air. Foles, in his four games didn’t throw any. And the team record for the year was 6 and 10, fourth in the AFC south which means last.
Like I mentioned, it may not be him or it may be a combination of everyone. But there is a question. So he may not be the problem or just part of it. But he may be the best part of the problem. He can’t look up after getting the ball and see a team photo of the other team and get yards.

Campbell’s problem was he was truly one dimensional when it came to scoring. In an 8 year career with two teams he scored 74 touchdowns. All of them on the ground. In the early 80’s, the west coast offense was coming in and backs were expected to have hands and catch the ball downfield. Career he had 121 catches for 806 yards, a 6.7 yard average. He became a dinosaur with his talents and injuries caught up with him with his crash running style.
He never talked about them, but he had them. After he retired, he had both knees replaced and four back surgeries. He also had to go through rehab for pain pills he took for nerve damage. He didn’t get all that in the whirlpool.

Actually, I have to admit I enjoyed watching the 49ers run the ball. The Titans were fun to watch at times too.
