Yes, it is his fault. Each actor is responsible for checking their weapon and SHOWING the gun is empty to the other actors and crew before the scene. It’s called professional courtesy and a safety check. Always done prior to filming.
If you have a gun in your hand; you’re responsible.
If you pull the trigger of the gun in your hand; you’re responsible. <—— notice the period.
The armorer may be responsible for the weapons on set, but the person holding it is the last and final check. It’s always been thus.
