deaths / cases is radically lower than deaths / infections, because there are huge numbers of asymptomatic, minor and otherwise untested infections not counted among documented cases.

The difference is in assessing what “huge” means. 50% additional is a huge difference. So is 30% So is 600%. So is 1000%. Which is it? I can’t say what it is in Italy, but I can say that in the US, until the breakout which showed this week the evidence suggested it was at the lower end of that scale.
