Pay your own way
The vast majority of people would not be able to pay for anything significant. A set of X rays and CT scans following a motor vehicle accident would bankrupt most Americans. If the scans showed something like a brain bleed or spinal cord injury or a lacerated organ, there would be very few people that could afford to pay and you would be left to die of your injuries. so we end up back at death panels and on a massive scale.
