Being in Texas I would suggest cross referencing the symptoms with Mexico and Central America. Probably an obscure disease in the US but rampant and imported from the South.

It’s been a long time, but it seems similar to my son’s scarlet fever when he was two. Scarlet fever is untreated strep, but sometimes initial strep tests come back negative. Get him checked again.
Has he been taking any prescription (or OTC) medications prior to this? Certain ones have a rare side-effect called Stevens-Johnson Syndrome, though other things can cause it:

Is there a change in anything that occurs in the afternoon but not during the rest of the day? Eating the same snack, drinking the same type of drink? Family member gone all day coming home? Alone with anyone at the same time of day (Munchausen by proxy?), around pets in the evening but not during the day? Clothing? Detergent? Soaps? Outdoor playtime?
Stevens-Johnson. Reaction to the antibiotic. I had a severe reaction to Cardizem. Rash stared around my waist then spread over my entire body. I looked like a burn victim. Took nearly a year for all the scarring to go away.
