Lots of missing information on all the plans listed; Is there a network involved, if yes, is your doctor in that network, if no, are there out-of-network benefits, what is the deductible amount for out-of-network, how much of the fee for out-of-network is applied to the deductible, etc.
Even if it’s in network, what does the phrase “of the cost” really mean - is that of the doctor’s contract rate for that particular code?
