The images I have seen here say “COLB” at the top. Not b.c. If there IS a “long form” this ain’t it. You can call a zebra a goose ‘til the cows come home but when you go to get feathers fer yer pillow you are gonna be disappointed.
Going through our box of family documents, I see that most of the birth certificates we have say “Certificate of Birth” at the top yet they are long forms.
