No those are not the only differences. The DeCosta BC looks very different than mine, have not looked at the other. The DeCosta border and print, plus some words are different like “accepted” versus “filed.”

Do YOU, Fred Nerks, know who Ms. DeCosta is? You must, because you believe her and/or the document supplied on her behalf without question.

What might be helpful, in the absence of your documentation, is if you were to enlighten us on how you applied for and obtained your copy of the FILED DOCUMENT.

It would be interesting to know what information you were required to supply to the Hawaii Health Department to obtain your copy. Obviously, a copy of your original birth certificate was NOT a requirement...for example, were you asked to stipulate the race of both parents?

...or, provide copies of THEIR birth certificates?

Just how easy or difficult is it, to obtain such a document from the Hawaii Health Department?
