Doctors: Warmbier suffered "severe injuries to all areas" of his brain
Doctors treating an American who was released earlier this week by North Korea said he suffered "severe injuries to all areas" of his brain while he was in captivity.
Speaking at a press conference Thursday, Otto Warmbier's doctors said he is breathing on his own but does not consistently respond to verbal cues. Warmbier, who was in a coma when he was released, suffered "extensive loss of brain tissue in all regions of the brain," one of his doctors said.
Doctors Daniel Kanter, Jordan Bonomo and Brandon Foreman briefed reporters on Warmbier's condition.
The doctors would not comment on Warmbier's prognosis or his chances of fully recovering, citing his parents' wishes. They described his current condition as being in a state of "unresponsiveness wakefulness."

Over zealous guards. Maybe Warmbier was so freaked out by how he was being treated that he caused himself a heart attack?

I don’t believe they would have used him as a testing subject in the way you explained. Not when they have thousands in their camps they do that sort of thing with all the time. Why would they damage the merchandise they detained/kidnapped for the deals they would be demanding for his return??.....
Kim surely wasn’t going to risk finding out so he simply offloaded the kid ....
Whole thing is an unfortunate and miserable happening for all his family......they just found out last week he was comatose only to find out this week it’s so much greater than this.
They’re still in shock and you can be assured on an emotional roller coaster....just a horrible happening all the way around.
Which could also explain why he had a heart attack, if he did.

Don’t really want to give the Norks the benefit of the doubt, but attempted suicide is a possibility. But the Norks have to know that even if that happened no one would believe it so they clam up and play dumb.

The botulism claim was very suspicious. Botulism causes paralysis of muscles, not coma. It kills by paralyzing the diaphragm, which causes suffocation. Had Warmbier been immobilized by botulinum toxin, I doubt he would have survived to be returned to the US.
They didn't want Warmbier to die in their custody
From what I'm reading, Warmbier has intensive damage to his brain, what does that mean?
Is he going to be a vegetable or unable to ever wake up?

Which is completely inconsistent with botulism. A person with botulism would be placed on a respirator and (possibly) given antisera to help them recover. The recovery can take weeks or months. The damage occurs to muscles because the nerves are prevented from relaying messages.
He probably just fell out of his comfy bed.
If you don’t wake up within a week, from a coma, your prognosis goes straight down hill.
Barring a miracle, this will be his life until he passes.
Do we know how to steer small, yet sufficiently impactful asteroids so that they’d collide at high speeds within a few thousand feet of their intended targets?
It's sounds like anoxic brain injury, from cardiopulmonary arrest - as you say, but this wasn't from a ‘heart attack’ (occlusion of a coronary artery) in an otherwise healthy 22 year old. They either overmedicated him with something that caused him to stop breathing, or they tortured him in some way. Even water boarding, taken to the extreme, could do this if they went too long and essentially induced the equivalent of drowning. There are a ton of things they could have done to cause this, but none of them are acceptable. Did they really think he was a spy? Sick.
Need to turn the spin cycle down on that brain washing. Retribution should be had in blood.

But there are high risk areas on the planet it makes sense to avoid if you want to increase your odds of not falling into the hands of evil people - Iran, North Korea. Somalia, etc.
