Who gets to judge “quality of life”?

North Korea makes the decision based on loyalty to the dictator, if your family had someone in prison for a political crime and you were lucky enough not to have been thrown in the concentration along with him (they often throw whole extended families into the camps, separated of course) you will find yourself with no benefits from the state, no job, no home and no recognition, you are "the unwanted", I think the word is "Kobechi"

These are the homeless people in the fields and mountains who collect and eat grass in order to try and stay alive. What to say to a 20 year old girl, her parents already died, suffering malnutrition?

Would you say her quality of life is too low and go ahead and die? Because that is exactly what was going to happen anyway, so it would be pointless.

North Korean spends about a dollar a year per capita on medical care and most North Koreans never see any of it, of course.

My point is let us not emulate this stuff
