In San Francisco in the 1970s and 1980s, it was routine to see men from China spit in the streets, blow their noses in their fingers and then wipe their hands on anything nearby, and generally violate American hygienic norms. Outside of the tourist zones, Chinatown's restaurants and grocery stores also suggested resistance to American hygiene.
The Chinese who came to America to escape communism were amazing people and model immigrants. They worked hard and were so family-focused and education-oriented that, usually within one generation, they made the leap from Chinatown squalor to lovely suburbs. However, unlike the Japanese, the Chinese did not bring with them a culture of cleanliness.
When it comes to epidemic diseases, these cultural norms matter -- and political systems may matter even more.
One of the things noted here last week is that coronavirus, unlikely ordinary respiratory viruses, may also be transmitted through fecal matter (emphasis added):
Speaking of filthy, one of the problems with coronavirus is that, even though it's an upper respiratory disease, it's also spread by fecal matter. It will be a disaster in places that don't have good fecal matter control: China (primitive toilets and no culture of hand-washing); India (which is working hard to bring toilets to people, but it's slow going); Africa (a world drowning in fecal matter); and San Francisco (also drowning in fecal matter).
That reference to hygiene in China gained new meaning from reading a viral post from Regie Hamilton, who was in China eighteen years ago to adopt his daughter. He vividly remembers the cultural comfort with fecal matter and other disease vectors:

You want filth and disease ridden?? Don’t look at China, look at our own Blue cities like Sh*t Francisco. The disease ridden “homeless” that we have failed to lock away are disease carriers and will spread this pandemic far and wide. Expect America’s cities to become hell-holes.
This is a flawed analogy. Look at Iran. Persians are better than Americans in hygiene except for the few mosque kissers

One would think that any highly populated country would take steps to limit plagues, but apparently one would be wrong.

Having lived in Japan 3 years and traveled quite a bit though China, Taiwan, Hong Kong...
The Japanese fixation with cleanliness and hygiene is a real thing. I don’t think I ever came across a sketchy food vendor.
Food hygiene in Hong Kong, but especially mainland China, at times was appalling. I would skip meals because what was available was just too questionable. That being said, stick to higher end places and you’re typically fine. Be picky.
These are contributing factors to those over 50 being more prone to having compromised immune systems.

I specifically mentioned that on Reddit and surprisingly, a good number of Redditers actually agree with me. Decades of uncontrolled coal burning for power plants in urban and industrial areas and too many heavy smokers in the country contributed to making the population highly vulnerable to COVID-19.
Re Asia.
Japan—Extremely clean. I would live there if I could speak and write the language.
Korea—Pretty close to Japan standards. All the food is safe to eat and you can drink the water.
Taiwan—Also safe and clean. I think a large part is they were part of the Japanese Empire until 1945, are still close to Japan, and follow a lot of Japanese culture and standards.
Mainland China—Don’t drink the water, watch out for the food. Real Chinese food is pretty horrible, a lot of overcooked boiled cabbage.
Vietnam—Same
Cambodia—Really dangerous, unless you are at a 5 star western resort.
“Traditional” Chinese medical beliefs also seem to have widespread acceptance there. For some reason, even in the West, Chinese superstition is treated with a dignity that is (rightly) withheld from traditional Western superstitions.
Many of the same people who would rightly think you were crazy if you underwent bloodletting in order to release the excess humours would go to an acupuncturist without a second thought.
Might be why we haven’t seen an explosion of cases here even though we have tons of international flights in and out of the country every day
If anybody has any common sense here in the US, we’ll use this moment to launch a giant public sanitation campaign - which would include bringing back laws against public defecation, spitting and trash, as well as vagrancy and public nuisance laws to break up the giant homeless encampments and send this population off to mandatory treatmentt...and get them and their waste out of the streets and doorways and transit hubs of our cities.
