In California, all the low income folks will go in MediCal, so there’s your single payer system right there. Then, as an individual’s income passes the federal poverty line and increases, that individual will have to buy insurance and the premium subsidies kick in. The subsidies last to three times the poverty line. Except for very few, the subsidies will not cover the entire premium.

Note that the point where the transition from “free” MediCal to payment with a subsidy kicks in varies from state to state.

It doesn’t really track with paying taxes. The requirement to have insurance is tied to income. Lots of the folks who don’t have a tax liability because they have several kids as writeoffs will bump into the insurance requirement because of their gross household income.

But I take your point in general, the subsidies are being underwritten by those who pay full premiums.
