Researchers have found that the association between heavy coffee consumption and kidney dysfunction hinges on a common genetic variation.
The researchers showed that markers of kidney dysfunction were nearly three times higher in heavy coffee drinkers with a variant of the CYP1A2 gene that makes them slow metabolizers of caffeine than for other heavy coffee drinkers who had a different version of the gene that enables faster caffeine metabolism.
"We think fast metabolizers can eliminate caffeine from their systems more efficiently and avoid harmful build-ups," said Ahmed El-Sohemy. "These individual differences in caffeine metabolism help explain why previous studies on coffee and kidney disease have been inconsistent."
The amount of caffeine a person consumes also appears to be important. For the current study, risk of kidney dysfunction was only significant in people who drank three or more cups of coffee a day, which is about 300 mg of Italian espresso. Current guidelines in Canada and the U.S. recommend no more than 400 mg per day for healthy adults.
The researchers also found that prevalence of the CYP1A2 gene variant that makes people slow metabolizers of caffeine was similar in both the study group and the general population: roughly 50%.
Many companies and clinics now include CYP1A2 in genetic tests.
"Heart disease, prediabetes and hypertension are all affected by variations in CYP1A2, which can also alter athletic performance," said Sara Mahdavi, lead author on the study and a former post-doctoral fellow in El-Sohemy's lab. "We can now be confident that whether or not coffee is deleterious to kidney health depends, in part, on CYP1A2."
The researchers studied three markers of kidney dysfunction: albuminuria (too much of the protein albumin in urine); hyperfiltration (high glomerular filtration rate in the kidney); and hypertension.
Estimates put the prevalence of kidney disease at about 13%.

From watchstanding on a ship I became a heavy coffee drinker.
Drank lots of coffee all my adult life, never any kidney problems. Got vaxxed and I now have kidney problems directly related to the vaxx.
I don’t believe anything the experts say anymore. Mostly because they usually are serving someone’s agenda.
I wonder what percent of those who have the genetic variant also had the vax.
How many Oz’s in a cup? I drink 48 a day religiously!! And if I skip that afternoon 24oz?? Pounder headache from hell!!
I do not drink coffee for my health but for the health of those around me.
Especially my young employees.
A pot of coffee suppresses my desire to beat their cheery butts.
I have one of each allele. What to do?
They just won’t leave people alone.
Bkmk
Good question. The two part bio-weapon has affected people differently.
“And if I skip that afternoon 24oz?? Pounder headache from hell!!”
I hear ya! The only headache I’ve had worse than coffee withdrawal was the monster from Covid.
I hope I get my mom’s genes in this regard. She’s 98 and has drunk strong black coffee, all day long, for 80 years.
btt!!
I was just carted off to the hospital with Congestive Heart Failure. Medical team of cardiologists there was absolutely terrific. Hospital food not bad, but they served decaf in the morning.
When I was discharged I asked about coffee. I drink half-caf at home, two cups in the morning. Head cardiologist said that was fine, it equalled one cup of coffee, an OK amount. I’m small, weigh 116, 5’2”, perhaps a larger person could tolerate more. Ask your own doc.
Three cups a day?
I’m gonna need a bigger cup!
With most of this stuff, RE: coffee, alcohol, etc — use common sense and moderate your intake.
