Since there is no legal definition of race, ‘hispanic’, or the other categories, it is impossible to answer the question correctly.
Let the gov issue strict definitions and we can answer properly.

But central planning needs to know the answers to these questions so it can place you in the proper high rise government housing complex with a 20 minute walk to your job if your health will allow it. Don’t you care about the environment and taking no more living space and wages than your fair share? Shame on you.
You may safely throw it away. They may harass you a bit with a visit or two or a phone call, but nobody has been prosecuted for refusing to co-operate.

I put mine in a briefcase and used Caller ID to screen many, many calls from them. Mysteriously they all stopped about the time the Lois Lerner IRS scandal broke. Have not heard from them since.

Since there is no legal definition of race, ‘hispanic’, or the other categories, it is impossible to answer the question correctly. For Race, I always say that I don't know. Because, honestly, I do not know the racial origins of my great-grandparents. I don't even know their names! I could well be African American, or any other race, and not know it.
