It says it is REQUIRED by law - but there is no way I am doing this. It's 28 pages of invasive, nanny state interfering that would shock our founding fathers.
Most of the questions have to do with my health or my insurance.
It wants info on each person, and then "What is your health insurance? Do you have disabilities? What is your job? What is your income? How much money did you make? Were you self employed? Really invasive questions - a nightmare - It's awful! Even a liberal would freak out!
Throw it away....claim you never received it....
Can you scan the form and post it? I’d like to take a looks at it.

I did that with the Census invasive form.
They sent a GS-5 to my front door.
You’ll begin to see all sorts of shady insurance sales scams with the advent of Obamacare. This is probably the tip of the iceberg that you’re seeing.
Although the law authorizes the Census Bureau to fine you $100-$5000, it has never happened.
Of course, you always run the risk of being the test case.
What I would *like* to do with such a form: Send it back with a note that tells them to call the NSA ‘cause they seem to know everyone’s business anyway and that I am too busy trying to survive to answer their d&^& survey.
I got one back after the census. When I didn’t send it in they called repeatedly and threatened me with “fines”.
Finally, I filled it out with completely fabricated info and sent it back.
We all know what it is going to take to stop this. And it will be very ugly if it happens.

Contact Glenn Beck, Limbaugh, Hannity, and Levin about so it will piss off both the right and the left. I’m pretty sure the left wouldnt want to answer those wuestions either.
Draw pictures of your dog on it, get a few coffee stains on it, answer “don’t know” wherever possible and contradict earlier answers where possible. Try to sound as stupid, stoned and confused as possible. Mail it in with 46 $.01 stamps on it even though it is postage paid.
They’ll figure you’re a democrat and leave you alone.
Tried that. They sent it again and then followed up with phone calls and threats of fines.
Just keep in mind that there is also the unspoken threat of a never ending existence in the cross hairs of the IRS.
These people are getting a lot ballsier now that we have The Fascist in Chief.
I’m sure he walked to the doors, I hope he limped away.
If they call you to ask about it just tell the pole smokers to use your NSA dossier and fill it out themselves.
