The thinking has been that strep throat bacteria trigger the production of antibodies that end up not only targeting strep, but mistakenly acting on an enzyme in the brain, which is involved in making brain chemical messengers. In so doing, the antibodies to the strep throat infection pathologically alter the balance of these chemical messengers.

Just had my tonsils out last month after decades of yearly strep bouts. Maybe I won't be such a neat freak anymore.
The author, a physician, recommends “vigorous treatment with antibiotics” if OCD patients are found to have antibodies in their systems (via spinal tap, ouch).
The strep is most likely long gone from the system then - the antibodies lingering, and antibiotics wouldn’t help.
My son has OCD, and Asperger’s, so this is interesting.

Could we at last have the explanation of why apparently intelligent people do suicidal things, like vote for Demonrat candidates, including totally unqualified "community organizers" with concealed personal records? Would antibiotics have saved America from communism? Stay tuned.

I had some OCD as a child. I was never diagnosed but I would repeatedly count steps so that I would take the same amount of steps per foot. I counted and made patterns out of very mundane tasks that had to be performed a certain way.
I most likely looked insane in front of a door taking extra steps with either foot so they would be even. I still catch myself doing very minor forms of OCD tasks, but nothing as severe as when I was a teen.
I never had lots of strep but I had ear infections severe enough to cause damage in both ears but the left one is particularly bad.
I’ve read other studies which seem to show OCD may be genetic, and related to other anxiety disorders. So for instance, one’s mother might have had Generalized Anxiety Disorder, and then you might have a combo of OCD and panic attacks. Of course, that doesn’t rule out it being genetic, with a triggering event such as a major infection required. Hard to say.

I don’t believe it. But I can’t debate it now, I have to get back to putting my wife’s cooking spices in alphabetical order, after I check the lint trap in the dryer. I can’t sleep knowing that it hasn’t been emptied.
