Here’s the video of the explosion. At about 3:20 you do see the upper stage explode. Have to wonder if the Flight Termination Sequence was activate.

The first stage engines kept firing and the first stage remained intact at least a couple of seconds after the disintegration.

On the YouTube video, they were still showing the graphic about the region of maximum aerodynamic stress at the time of the anomaly.

It also looks to me like the capsule survived and was tumbling through the debris cloud, although it could have been some other part of similar size.

I wonder if the first stage engines didn’t throttle down correctly, the speed could have put too much pressure on the upper stage.

It was at maximum dynamic pressure or just after. Did they fly it apart?

It actually was past Max Q.

I’m think the RSO initiated the Flight Termination System or the upper stage had a major problem. It’s still possible there could have been a structural failure that didn’t show up until after Max Q.

It looked like the second stage disintegrated with no ignition. Perhaps the fuel tanks were being brought up to pressure in preparation for staging, and they failed.

They were getting close to staging. Do you know if the second stage tanks have to be pressurized or some other preparation for staging could have caused the failure?
