He likely went under the front left wheel and his head got caught up in the axle assembly or wheel well. Part of the lower body gets pinned under the moving wheel, causing his head to be pulled off by the forward movement of the truck.

Funny, I don't see anything else in the street but the covered head. There's a crosswalk at the intersection. I wonder if that's where he got hit, and his head landed there. That truck in the background is apparently the one that hit him. There's a picture on another site showing NYPD investigators hunched down looking underneath the truck with flashlights, but there's nothing on the truck either.
