Trump is my man and I am supporting him just as I always have, but I swear I wish he would get off that damn name calling. It belittles him more than the one labeled.
And why give a nickname to someone that the general public has never heard of? It’s just weird. Makes no sense.

I think you may have hit the nail on the head.
One of her family’s ships was stopped with a huge haul of Cocaine on it in Latin America right before Trump began using the phrase.
I usually assume Trump knows stuff we do not, and take it to mean that behind the scenes her family has some relations with the Cartels, or maybe some drug shipper like the CIA, and they pad their income that way.
I’ve gotten a glimpse behind the curtain, and you have no idea the degree to which MS-13, FBI, the Mafia, the Cartels, the CIA, Big Business, Big Media, and the US political scene are all under the same command/management, and working together.

Yes, Rightwing Conspiratr1’s interpretation seems the most likely of all put forth here. That it also carries the connotative meanings of the other interpretations makes it nothing less than brilliant.
