As you describe, path also makes a difference.

Back in the day when I worked with Houston Looting and Plunder (Lighting and Power), we had a well experienced lineman killed off 240V.

Hot sweaty day, he was squatting in front of a Padmount Transformer, energized and open to the termination cabinet.

As he started he started to stand, he fell forward. The termination pads impacted on the left and right side of his chest.

He was given immediate CPR by others on the crew; they were all CPR trained. City area and the ambulance was there in approximately 5 minutes from contact. He was never revived.
